This form is to CANCEL a care visit (or visits) for a short time (not exceeding 1 week). Please be aware, that once you have submitted these dates, the visits may not be possible to reinstate if you change your mind later (though we will always try our best to reinstate where possible.)  When canceling visits, please take into consideration a person's medication, food, drink, and other important welfare needs/tasks that MoralCare usually completes. Please make sure you have alternative arrangements in place so your loved one continues to have their needs met and welfare maintained whilst their place is on hold.

Please note: Temporary cancellations are charged at 50% retainer fee, unless it is a bank holiday, where no charge for cancellation will be incurred.
